Pleomorphic adenomas (benign mixed tumors) are the most common nonvascular benign salivary tumors in children (Fig. 56-6).42,54 They present as firm, rubbery masses, most often in the parotid gland, with an average age at presentation of 9.5 years within the pediatric population.54,55 The tumor presents as a painless, slowly growing mass and is rarely infiltrative.56 They have variable echogenicity on imaging, with increased heterogenicity seen in larger lesions secondary to necrosis or cystic changes.4 Treatment of superficial lobe tumors includes superficial parotidectomy with facial nerve dissection and preservation. Recurrence rates have been reported to be up to 40%; so, long-term follow-up is recommended.57,58 Simple excisional biopsy should be avoided, because it is associated with a higher recurrence rate. Rarely, recurrent pleomorphic adenomas may undergo malignant degeneration.59

The total cumulative dosage of radiation therapy that is administered for pleomorphic adenomas is 50 to 60 Gy depending on the particular surgical and pathologic characteristics of the case.Kim and Lee reported that enhanced, multidetector CT scanning with a 50-second delay between contrast injection and scanning can be used to differentiate Warthin tumors from pleomorphic adenomas and malignant tumors. [10] The etiology of pleomorphic adenoma is unknown, but the incidence of this tumor has been increasing in the last 15-20 years in relation to the exposure of radiation. One study suggests that oncogenic simian virus (SV40) may play a role in the onset or progression of pleomorphic adenoma.  Prior head and neck irradiation is also a risk factor for the development of these tumors. [4]

MR Imaging Features of Basal Cell Adenoma of the Parotid Gland

Pleomorphic adenomas (the most common benign parotid tumor) are typically well defined or multilobulated masses. Because 90% of pleomorphic adenomas occur in the superficial parotid, they are usually assessable with ultrasound, typically showing a hypoechoic mass, often with through-transmission and hypervascularity. They are normally hyperdense on CT relative to the parotid gland, but may be isointense and therefore difficult to appreciate.59 On MRI, an appearance of T1 hypointensity and T2 hyperintensity (because of the fibromyxoid stroma) with avid, homogeneous enhancement is considered strongly indicative of pleomorphic adenoma. There may, however, be heterogeneity in the presence of cystic change, hemorrhage, or dystrophic calcification, and these tumors may not be distinguishable from malignant tumors. Long-standing lesions are at risk of malignant transformation. Differentials for pleomorphic adenoma include Warthin tumor, parotid nodal metastasis, facial nerve schwannomas, myoepitheliomas Kato H,Kawaguchi M,Ando T,Mizuta K,Aoki M,Matsuo M, Pleomorphic adenoma of salivary glands: common and uncommon CT and MR imaging features

Such cells can cause concern about malignancy.31 In addition cystic changes can also lead to diagnostic difficulties since such lesions may show atypical squamous cells and even mucous-producing cells (Fig. 22.11). However, in such cases any features suggestive of pleomorphic adenoma should overrule occasional findings that may be suggestive of malignancy. This is the “dogma of pleomorphic adenoma,” which is practiced in our clinic.

Pleomorphic adenoma, the most common salivary gland tumor, is also known as benign mixed tumors (BMT's), because of its dual origin from epithelial and myoepithelial elements. It is the commonest of all salivary gland tumors constituting up to two-thirds of all salivary gland tumors.[1][2][3]Pleomorphic adenomas (see the images below) are typically smooth, well-marginated tumors, although nodularity along the outer surface is sometimes present. The attenuation values of the mass are usually homogeneous and higher than that of the surrounding gland, although lower attenuation masses that resemble cysts are occasionally seen. Pleomorphic adenomas have a female predominance (2:1) and typically affect patients between 30 and 60 years of age. [13]  Pleomorphic adenomas are associated with a 2-25% risk of malignant transformation over time, and recurrence rates have been shown to be higher in patients who undergo enucleation. [19]  CT is not a sensitive test for salivary tumors and is not routinely performed. Pleomorphic adenomas are homogeneous and well defined with moderate delayed contrast enhancement; small lesions may be missed without delayed postcontrast imaging. The tumor is usually higher in attenuation than the normal gland but may occasionally appear cystic. Large tumors are more likely to have heterogeneous attenuation (due to necrosis and hemorrhage) and a lobulated contour, which is highly suggestive. Recent hemorrhage may result in an ill-defined outline on CT, thus mimicking malignancy. However, MRI tends to confirm the benign nature of the lesion.2The most common type of salivary gland tumor is a slow-growing noncancerous (benign) parotid gland tumor, which gradually increases the size of the gland. However, some of these tumors can also be cancerous (malignant). The most common benign tumor is pleomorophic adenoma, and the most common malignant tumor is the mucoepidermoid carcinoma. Mucoepidermoid Carinoma is an abnormal growth of tissue resulting from uncontrolled, progressive multiplication of cells, serving no physiological function. This is the most common type of salivary gland malignancy in adults, and most of them appear clinically as so called mixed tumors.Malignant transformation within a pleomorphic adenoma (carcinoma ex pleomorphic adenoma) can occur. It is then most often an invasive, undifferentiated carcinoma with markedly abnormal malignant cells and will thus not cause diagnostic problems. More differentiated malignant tumors such as adenoid cystic carcinoma and mucoepidermoid carcinoma have also been reported.

Gadolinium-based contrast agents have been linked to the development of nephrogenic systemic fibrosis (NSF) or nephrogenic fibrosing dermopathy (NFD). The disease has occurred in patients with moderate to end-stage renal disease after being given a gadolinium-based contrast agent to enhance MRI or MRA scans. NSF/NFD is a debilitating and sometimes fatal disease. Characteristics include red or dark patches on the skin; burning, itching, swelling, hardening, and tightening of the skin; yellow spots on the whites of the eyes; joint stiffness with trouble moving or straightening the arms, hands, legs, or feet; pain deep in the hip bones or ribs; and muscle weakness. The ratio of epithelial-to-mesenchymal components varies widely. Some pleomorphic adenomas may be quite cellular, composed almost entirely of epithelial or spindly myoepithelial cells, whereas others have predominantly mesenchymal components with very few epithelial cells. The variability in histologic pattern is typically reflected in FNA.Evidence suggests that dual-isotope imaging with technetium-99m and thallium-201 single-photon emission CT (SPECT) is accurate in distinguishing various tumors of the salivary glands, including pleomorphic adenomas.

However, there is evidence to support the more conservative approach of extracapsular dissection (Fig. 56-5). The main difference between partial superficial parotid resection and extracapsular parotid dissection is identification and dissection of the facial nerve and removal of a margin of uninvolved glandular tissue.42 Several authors have shown that partial parotidectomy and extracapsular dissection of a benign pleomorphic adenoma can be performed with comparable rates of local recurrence.42,43 However, Witt and Rejto compared extracapsular dissection with partial superficial parotidectomy with respect to tumor recurrence and transient and permanent facial nerve dysfunction.44 They performed a literature review with statistical analysis of published series and found a significantly higher rate of recurrent pleomorphic adenoma and permanent facial nerve dysfunction and a lower rate of transient facial nerve dysfunction with extracapsular dissection than with partial superficial parotidectomy.44Warthin’s tumor is a type of benign tumor of the salivary glands, and the gland most likely affected is the parotid gland. Only pleomorphic adenoma is a more common benign parotid tumor. Warthin’s tumor is slow growing, painless, and primarily affects older individuals. The tumor is bilateral, and highly unlikely to become malignant. Some authors have reported excellent sensitivity with ultrasonography, although ultrasound typically does not help the surgeon understand the 3-dimensional (3D) relationship of the tumor to the parotid gland and the facial nerve. [6, 14, 7, 15, 8, 16]  Ultrasonography is heavily operator dependent, provides limited visualization of the deep lobe of the parotid gland, and often has difficulty distinguishing benign from malignant neoplasms. [13]  On sonograms, pleomorphic adenomas typically appear as smooth, round, hypoechoic masses with distal acoustic enhancement. [9, 17]  Lobulations are commonly visualized. Large tumors appear more heterogeneous than small ones and are better imaged with CT or MRI than with sonography. [18, 15]

Went to the doctor because I was having lots of issues with my menstrual cycle. Saw 4 specialists and finally was told that I have a tumor in my head called an adenoma pituitary tumor

Histologically, pleomorphic adenomas have a capsule that often is incomplete, a fact that explains recurrences after apparent complete excision. The tumors are composed of epithelial and myoepithelial elements and a sparse to abundant stroma. The epithelial component may have a trabecular, glandular, solid, or adenoid cystic (cylindromatous) pattern. Foci of squamous metaplasia, oncocytic changes, mucous production, and cells with sebaceous differentiation may be encountered within the same tumor. The stromal component can be myxoid, chondroid, or myxochondroid. [5] Pleomorphic adenomas are benign tumors of salivary glands, but they can also occur in lacrimal glands. The pleomorphic adenoma is the most common benign tumor of the parotid glands and accounts for 75% of all parotid epithelial tumors. These adenomas occur rather frequently and generally are found in a relatively young population.Pleomorphic adenoma of the breast is a rare benign tumor, with patients ranging age from 23 to 78years.206 Clinically, pleomorphic adenoma presents as a firm, nontender, mobile mass in the subareolar area. The mammogram can reveal a mass with poorly outlined borders that suggest a malignant process. Pleomorphic adenoma of the breast shows cytologic features identical to its more common salivary gland counterpart.206–211 The absence of bipolar naked nuclei and spindle cell stromal fragments are helpful in differentiating this entity from fibroepithelial lesions. However, the presence of noncohesive epithelial cell clusters, atypical isolated cells with high N/C ratios, mild pleomorphism, and reticular chromatin with prominent nucleoli may lead to false-positive diagnosis, such as metaplastic carcinoma or phyllodes tumor.206,207,211,212 Pleomorphic adenoma, which is also known as mixed tumor, is a benign tumor composed of a A computed tomography (CT) scan usually shows a well-circumscribed tumor with lobulation. The tumors usually cause a firm, painless swelling in one of the salivary glands, and the size of the swelling gradually increases. Sometimes, these tumors also cause facial nerve palsy, which makes it difficult for patients to move one side of the face. Pleomorphic adenomas can vary in size, depending on the gland in which they are located. In the parotid gland, the tumors are usually several 9. CT image shows a lobular mass in the left parotid gland. The circumscribed borders and location are highly suggestive of pleomorphic adenoma.. Warthin tumors (cystadenolymphoma, cystadenoma lymphomatosum papilliferum) account for approximately 10-15 % of all benign salivary gland tumors and are Though much less likely to occur than pleomorphic adenoma, Warthin's tumor is the second most common benign parotid tumor Copyright © 2020 Center for Advanced Parotid & Facial Nerve Surgery. Microscopically, Warthin tumor is characterized by bilayered oncocytic epithelial cells arranged as papillary projections lining cystic spaces

Pleomorphic adenomas of the minor salivary glands, like those on the palate, buccal mucosa or lip, sometimes lack encapsulations and may mix into normal host tissue as tumor growth; hence a wide excision is necessary even if previous biopsies report benign nature Woo SH, Choi DS, Kim JP, Park JJ, Joo YH, Chung PS, et al. The use of postoperative radiation therapy must be based on selective and judicious indications, as noted earlier, as well as astute clinical judgment. Despite its effectiveness in controlling pleomorphic adenomas, its liberal administration is contraindicated. Because these lesions are benign and are usually found in young patients, the exposure of such patients to the small but definite risk of developing a radiation-induced malignancy as a late complication is unjustified.Pleomorphic adenomas harbor a small risk of malignant transformation. The malignant potential is proportional to the time the lesion is in situ (1.5% in the first five years, 9.5% after 15 years). Therefore, excision is warranted in almost all cases. Other risk factors for malignancy include advanced age, radiation therapy, large size, and recurrent tumors. PLEOMORPHIC ADENOMA. Inflammatory phase Differentials for pleomorphic adenoma include Warthin tumor, parotid nodal metastasis, facial nerve schwannomas, myoepitheliomas, mucoepidermoid and adenoid cystic carcinoma, and a large variety of other neoplasms nonspecific to salivary glands. Histopathology remains the gold standard in differentiating them all. Among salivary gland neoplasms, metastasizing pleomorphic adenoma (MPA) constitutes an extremely rare group of tumors. The present paper reports a case of pleomorphic adenoma (PA) in submandibular gland that, after more than 30 years of initial treatment, recurred and metastasized to..
